Please find attached the Alberta Checklist that need to be filled out for the children and the teachers/helpers that are teaching for the week.

Please write the child's name or teachers/helpers name on the top of the page with your temperature.

Please drop your checklist at the check-in station just inside the sanctuary.

Copies will be available at the check-in station.  Temperatures will also be taken there as well.
